An opportunity has arisen for a Nurse, Social Worker or Occupational Therapist to join the Blackburn & Darwen Community Mental Health Team. You must be a motivated individual, committed to promoting recovery and well‑being for our service users as part of our multi‑disciplinary team. Excellent communication skills are essential for the role.

We are looking to appoint a Band 6 Community Mental Health Practitioner. This post provides the opportunity for a forward‑thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care.

With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. You must be a registered mental health practitioner, with excellent leadership and clinical skills.

You must have some post‑qualification experience in clinical work with service users experiencing a range of mental health problems.

You need to be able to work as part of a multidisciplinary team and have experience of planning, implementing and reviewing care needs.

You will also need to have a working knowledge of the care programme approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well‑being.

Main duties of the job

• Regularly supported and supervised by the clinical lead/team leader, you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
• Undertake the administration of medications and safe storage of the same.
• Actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct.
• Maintain links with your professional body and provide evidence for the revalidation process.
• Provide support to the nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users.
• Exercise professional accountability and responsibility using skills, knowledge and expertise in changing environments, across clinical boundaries and in unfamiliar situations as outlined within the NMC Code of Professional Conduct.
• Operate electronic staffing record systems such as Appraisals/supervision as required.
• Maintain a positive and open attitude towards organisational change and service development.
• Work flexibly by covering the core community mental health team hours of Monday – Friday 9 am – 5 pm, to meet the needs of the service and the wider network.

Person Specification

Qualifications

• Registered Mental Health Nurse, Occupational Therapist or Social Worker
• Ongoing Professional Registration

Knowledge

• Understanding of health and safety policies and procedures
• Knowledge and practice of the Mental Health Act 1983
• Evidence of recent professional development
• Demonstration of how research has influenced practice

Experience

• Willingness to participate in mentor and preceptorship processes in respect of own practice
• Experience of assessing, planning, implementing and reviewing care needs
• Able to work as part of a multidisciplinary team

Skills

• Able to demonstrate and understand the concept of community care and the need to develop good networks
• Strong communication skills through practice and experience

Other

• Ability to travel within the organisational footprint to meet the essential requirements of the post

Salary

£39,959 to £48,117 a year/Per Annum (subject to confirmation)
