Overview

The Home First service brings together nurses, paramedics, therapists, health and therapy assistants to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient’s own home. Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post-discharge to prevent re-admission. Home First will, where required, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

You could become part of our exciting service, bringing the unique benefits of a wraparound recovery service from hospital discharge. The principle of the service is ensuring that recovery involves working towards the person being as independent as possible through enablement.


Role

We are looking for a Clinical Lead (Therapist) to join us. If you’re an ambitious, proactive, and experienced therapist looking to take a lead role in a team that provides cutting-edge, excellent community care, we’d love to hear from you!


Responsibilities

The post holder will:

* Provide Clinical and Professional Leadership, ensuring effective governance for staff working in Home First.
* Collaborate with the AHP Head of Service and Professional Leads to ensure staff are clinically competent, services are clinically effective and safe, and an excellent patient experience is delivered.
* Embed the principles of interdisciplinary working and cross-skilling for all staff, including implementing the competency frameworks.
* Provide specialist clinical advice regarding complex patients and deliver specialist clinical assessment and intervention for highly complex patients as required.
* Support an audit plan in the appropriate clinical area and participate in research as appropriate to inform future service development.
* Develop key partnerships and enable whole-system working through effective communication and cross-border collaboration with other disciplines.
* Ensure services continue to develop in line with Trust Strategy and act as deputy to the line manager.


Requirements/Qualifications

* Registered therapist with the relevant professional body (as applicable).
* Ability to provide clinical leadership and senior professional guidance within a multidisciplinary team.
* Experience in community-based care and interprofessional collaboration.
* Flexibility to work 7 days a week during service hours and across the geographic area.
* You will need to be able to drive with a full UK licence and have access to a vehicle due to the geographic area covered by the team.
