Overview
As a Transition Assistant, you will work with children and young people to promote their independence, with the aim of them being able to reach their full potential within their home and wider community. Some of the children and young people you work with may have special educational needs and disabilities (SEND). Based in Kingswood, the team sits within South Gloucestershire’s 0-25 Disabilities service, focusing on early interventions and supporting key transitions for those referred to the service.
Responsibilities
* Provide individual support to a child or young adult using a person-centred approach, contributing to them being able to access services that are available in their local community.
* Undertake regular reviews, risk assessments and visits to service users, offering advice and support to ensure that care meets the required standards.
* Support the individual with travel requirements or be involved in a school travel training project, ensuring vehicle fleet services meet workplace protocols and vehicle checks.
* Work with care groups and other agencies to share information and provide additional support.
* Maintain confidential reports and case notes, recording care plans and risk assessments in accordance with professional and council guidelines.
* Refer any issues in accordance with safeguarding procedures to protect vulnerable people.
Qualifications
* Hold an NVQ Level 2 or 3 or GCSEs grade C/4 or above, or have equivalent qualification or relevant experience in office administration.
* Demonstrate knowledge of relevant legislation, regulations, codes of practice, support mechanisms and specialist equipment.
* Have previous training and experience in a similar service environment.
* Ideally have experience managing challenging behaviours, e.g. behavioural difficulties in homes or the community.
* Beneficial to have knowledge of other agencies, professional support groups and related external organisations.
Requirements
* Work Wednesday 12:30pm-4:30pm and Thursday and Friday 08:30am-4:30pm.
* Be expected to travel throughout the authority and surrounding area, requiring a full valid driving licence with regular access to a vehicle or alternative means of travel.
* Business insurance must be added to your current insurance policy.
* Obtain an enhanced DBS for both Children and Adults as a requirement of employment.
