Advanced Clinical Practitioner role at Somerset NHS Foundation Trust – Hospital at Home Frailty ward. We have 2 full-time posts available; one set within the team. Are you a compassionate registered professional with advanced clinical skills seeking an opportunity to work within a hospital at home team and help patients stay in their own home environment within Somerset?
Service covers the urban and rural areas of Somerset, with cross-cover especially at weekends and depending on service needs.
Hospital at Home operates between 8am and 8pm with a variety of shifts in the rota. Flexible working solutions are offered where possible to support work-life balance. Weekend cover is required. 30 hours is the minimum contract hours for this role. Training and supervision are available to support and guide you in your new role.
Advanced skills such as non-medical prescribing are essential for this role. You will need to be a car driver with your own vehicle to visit patients; a generous mileage allowance is provided for NHS business, including both mileage and depreciation.
Responsibilities
* Provide advanced clinical care within Hospital at Home Frailty services, including assessment, planning, and delivery of care for patients in their home environment.
* Work as part of a multidisciplinary team, with cross-cover at weekends and as service needs arise.
* Utilise advanced clinical skills and, where required, non-medical prescribing to support patient care.
* Participate in the rota and ensure timely, safe, and effective patient care within the service hours of 8am–8pm.
Qualifications and requirements
* Registered healthcare professional with advanced clinical skills.
* Non-medical prescribing is essential.
* Car driver with own vehicle to visit patients; mileage allowance provided.
* Willingness to work weekends and flexible shifts.
* Ability to work within a Hospital at Home setting serving Somerset’s urban and rural areas.
