Job overview

The post holder will be an integral member of SBUHB’s Urgent and Emergency Care Improvement & transformation team and will play a key role in embedding QI culture across Swansea Bay University Health Board (SBUHB). An ambitious and highly motivated individual is required to provide high-quality Improvement support aligned with the 6 Goals for Urgent and Emergency Care framework. Specifically, this will include working on Optimising Patient Flow and Discharge to Recover then Assess (D2RA) initiatives. The facilitator will be imperative to ensuring the planning and delivery of key works across all service areas within SBUHB. The focus being on encouraging:

· Timely initial assessment of patients on admission (criteria to reside) and discussion(s) re; likely discharge date (to set early expectations).

· Utilisation of SAFER and Red2Green principles and timely recording of D2RA pathway at Board Rounds.

· Encourage and embed initiatives to avoid deconditioning of patients when in an acute setting

· Close working with social care/ Local Authority/ Third Sector partners (as part of Integrated Discharge Hub) to encourage timely flow and discharge.

· Other activity as determined by needs of the system e.g. PDSA cycles and monitoring of improvement activity.

This role will require a collaborative approach in working with colleagues both within and outside of SBUHB and provide a pragmatic approach to change.

Main duties of the job

•Develop measurement plans for improvement programmes and ensure that appropriate data is available to teams regularly in a format they can use.

•Expected to work autonomously daily, will exercise a high degree of personal initiative in carrying out their duties and will report to the Improvement Lead for discussion and direction.

•Assist the clinicians and Service Managers in the successful implementation and delivery of the Optimal Hospital Flow and D2RA schemes of work.

•Support planning, organisation and delivery of highly complex activities, which includes being flexible and dynamic to adjust plans and strategies to aligned to service delivery.

•Have knowledge of how to map and describe the current process pathways from a clinical and administrative perspective, identifying recommendations for changes to ensure an effective patient-centred system is in operation across varying clinical areas and specialists.

•Plan and prioritise own work, to ensure effective support to all areas and delivery of key objectives.

•Provide advice and support to the Programme Lead (Associate Director – Unscheduled Care) and other colleagues as required.

Qualifications & Knowledge

Essential criteria

1. Professional knowledge acquired through Nursing, Occupational Therapy, Physiotherapy Degree or equivalent experience or training.
2. Post graduate diploma or able to demonstrate the equivalent level of knowledge, skills or experience. Evidence of quality improvement practice within professional or voluntary roles, plus further staff or project management knowledge.
3. A relevant qualification in improvement methodology (including any organisation specific online/face to face training).
4. Evidence of continuing professional development. Able to effectively use Word processing, Excel, PowerPoint and Outlook

Desirable criteria

5. Knowledge of records management. Digital and data management, practice, storage and analysis and interrogation.
6. Knowledge and experience of optimal hospital flow & D2RA principles and policies.

Experience

Essential criteria

7. Experience of managing/supervising staff.
8. Experience of Quality Improvement, project management, service transformation or remodelling of services.
9. Experience of delivering presentations.
10. Experience of developing and delivering training.
11. Recent experience of working within an acute or community hospital setting.

Desirable criteria

12. Experience of financial planning and monitoring.
