We have an opportunity for a Permanent, Full Time Social Worker within our Adult Social Care Connect Team.

We want to ensure that our community and people who access our services feel safe, supported and connected enabling each person to live independently with opportunities to use technology‑enabled care and self‑serve wherever possible.
Adult Social Care Connect is the front door team for Southampton City Council Adult Social Care referrals alongside our corporate Service Centre. Its primary focus is new referrals into Adult Social Care where no social care provision is currently in place. There is a strong focus on early intervention and prevention and providing clear pathways to our short‑term and long‑term care services. This service also includes operational safeguarding and supports the delivery of consistent approaches to safeguarding across Adult Social Care.
The social work role and responsibilities include undertaking assessments under the Care Act 2014, Mental Capacity Act 2005 assessments, contributing to multi‑agency planning, providing interventions to individuals and their families or carers, negotiating and coordinating resources and conducting s.42 safeguarding enquiries.

Main Duties, Tasks & Skills Required
* To participate in the effective delivery of adult social care services in line with statutory responsibility and the principles of best value.
* To work with staff to ensure they are working in a strengths‑based, person‑centred way with people and their carers who require adult social care intervention, including having conversations about what matters most to them to complete and authorise strengths‑based assessments promoting independence, health, and wellbeing.
* To create and review outcome‑focused plans, negotiate for and coordinate resources to meet needs.
* To prevent harm and manage risk to adults in need of care and support in line with the principles of Making Safeguarding Personal by keeping the person at the centre of any planning and promoting positive risk taking.
* To help people build resilience and achieve outcomes that are important to them by ensuring that they receive accurate information and advice and guidance and connecting them to wider networks of opportunity and support.
* To work with colleagues and partner agencies to ensure a high quality, responsive social care service leading to positive outcomes for adults with care and support needs.
* To provide practice and professional leadership, support and supervision for social workers and social care staff in adult care teams, with specific focus on Mental Capacity, Safeguarding incl. MSP and Legal Literacy and positive risk management.
