We arelooking for a part-time (25 hours) Care Navigator/Receptionist to join ourfriendly, patient-focused team.
Thisposition is a permanent one and working hours will be over 5 weekdays, shiftpatterns to be confirmed.
Main duties of the job
Thesuccessful applicant will be:
* Experienced with SytmOne -Minimumof 1 year experience, 1 years customer service experience and must be able to demonstrate where this experience was obtained.
* Organised, efficient anddemonstrate good interpersonal skills and effective communication within ourmulti-professional Primary Care Team.
* You will need to have thecapability to prioritise your own workload, be confident and adaptable to bothchange and to challenging situations in a busy environment.
* You will also be able todemonstrate co-operative and productive working with people of all backgroundsand second language would be desirable for this role.
* You will need to computerliterate, experienced in the use of electronic communications and be confidentduring face to face interactions with patients.
* This post requires an ability toshow attention to detail even under pressure while maintaining confidentialityand the need to be able to deliver accurate and clear information toappropriate parties, in a timely manner.

Job responsibilities
* Interact with patients to provide and process information inresponse to enquiries, concerns and requests about NHS/practice services.
* To ensure that all patients have a positive experience.
* To work as part of the Patient Services Team in a professionalmanner to provide high quality service, administration andsupport services to all clients, visitors, GPs and Allied HealthProfessionals.
* To follow all standard operating procedures and policies to ensureworking in an efficient and courteous manner at all times.
* To provide a professional call handling service and associatedadministration duties.
* To embed and sustain an ethos of care navigation throughout theteam; providing advice and guidance and signposting patients to the most appropriateclinician/service.
* To record all essential information accurately.
Person Specification
Qualifications
* -GCSE grade A to C in English and Maths (or equivalent).
* -A minimum of 1 year experience with SystmOne.
* -Excellent communication skills, both written and verbal.
* -Competent in the use of IT.
* -Ability to deal confidently and courteously with people both in person and over the telephone.
* -Ability to work flexibly as a team member.
* -Able to work on own initiative and prioritise workload.
* -Empathetic, honest & caring.
* -Able to work under pressure.
* Experience in a patient facing/customer service role
Experience
* -1 Year minimum experience of working with SystmOne
* -1 Year minimum experience of working in a customer service role
* -Experience of working in a GP practice
