About the Role
This is an exciting opportunity for a qualified Clinical Pharmacist experienced in primary care and chronic disease management to deliver patient-centred care remotely.
You’ll play a key role in supporting safe, effective prescribing and helping patients manage their long-term conditions through structured medication reviews and follow-up consultations.
Key Responsibilities
* Conduct Structured Medication Reviews (SMRs) and medicines optimisation remotely.
* Manage patients with long-term conditions such as hypertension, diabetes, and asthma.
* Provide clinical support to GPs and the wider multidisciplinary team.
* Deliver patient education and ensure adherence to best practice and NICE guidance.
* Accurately document all activity using SystmOne.
What We’re Looking For
* Registered Clinical Pharmacist (GPhC)
* SystmOne experience is essential
* Proven background in primary care / GP surgery setting
* Confident conducting remote consultations
* Excellent communication and patient engagement skills
