To assist the Service Manager in leading a team, supporting individuals experiencing challenges with their mental health to develop the skills and confidence to move on to their own tenancy. To work collaboratively with the external agencies involved with the people we work with to move towards achieving their goals through holistic, social and emotional support. In line with the projects operational plan.
To ensure service excellence through motivation and inspiration of team members.
Liaise with the Service Manager and team to co-ordinate and manage the work of the projects in line with its aims and objectives.
To get to know people who are referred into Caerphilly Supported Housing to see if our service is right for the person. If so, to start supporting the person in moving towards independent living in a recovery orientated way with a trauma informed approach. Where our homes are not for the best support for the person to look into exploring alternative options.
To work with people who are stepping forward from Caerphilly supported housing into their own property to provide transitional floating support as well as tailored support delivered to people in our Star Flat accommodation.
To provide housing related support to people who experience challenges with their mental health and other areas of their life.
Support people to develop the skills to manage their mental health, emotional distress, finances, benefits, household and tenancy.
To provide support and mentoring to colleagues in the implementation of support plans and in delivering the aims of the service.
