An exciting opportunity to work at St Lukes Hospice, supportingthe Physiotherapist and Occupational Therapist working with patients in theirown homes, other community settings and the specialist inpatient unit.
The AHP team strives to provide palliative care to patientsin any setting to achieve the best quality of life by helping patients live asactively as possible and achieving their personal goals adding significant qualityto their lives.
Main duties of the job
The role of theAHP Support Worker would be to contribute to the assessment and treatment ofpatients both individually and in groups settings and support the Therapists todeliver treatment plans with patients, assist in the provision of equipment andcomplete assessments to support the activities of daily living.

To assist the qualified AHP indelivering Palliative Physiotherapy and Occupational Therapy services topatients in community setting and within the Specialist in-patient unit.
The post holder will work as partof an integrated multi-disciplinary working alongside the specialist teams providing supportfor patients with complex palliative care needs, both within the Hospice andpatients home environment.
