About the Role
The primary purpose of the role is to delivercoordinated comprehensive, and person-centred case management for individualswith moderate to severe mental illness.This includes assessing needs, developing and implementing care plans,connecting service uses with resources and monitoring progress to supportrecovery independence and improved quality of life.
About you
We invite Registered Health Care Professionals such as Registered Nurses/PsychiatricNurses, Occupational Therapists or Social Worker to consider this role, that haverecent relevant clinical experience within mental health. Experience with completing comprehensiveassessments, risk assessment and interventions for service users with moderateto severe mental illness is required for this role. Other success factor for this role includes;
* Current NZRegistration and APC
* MentalHealth experience in a similar setting
* Culturallyresponsive practice
* A Current New Zealand Drivers Licence is required

Salary Range
This role is covered by the terms andconditions of the relevant HCP Collective, and the Pay Scale, determined onrelevant experience.
