Job summary
The care navigator is the front door to the surgery, handling all calls and patients presenting at the front desk with a wide range of queries. Your role will be to support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team.
You will also undertake reception duties, supporting the administrative team in delivering a polite and professional service to the patient population.
Main duties of the job
The core responsibilities of the Care Navigator include, but are not limited to:
1. Processing and signposting patients to the appropriate healthcare professional based on their presenting condition.
2. Answering incoming calls, transferring calls, or handling requests appropriately.
3. Processing patient requests for appointments.
4. Initiating contact with and responding to requests from patients, team members, and external agencies.
5. Photocopying documentation as required.
6. Data entry of new and temporary registrations and relevant patient information.
7. Inputting data into patient healthcare records as necessary.
8. Directing requests for information, such as SARs, insurance/solicitors' letters, and DVLA forms, to the administrative team.
9. Managing queries efficiently.
10. Maintaining a clean, tidy, and effective working area.
11. Monitoring and maintaining the reception area and noticeboards.
12. Supporting clinical staff with general tasks as requested.
13. Undertaking all mandatory training and induction programmes.
14. Attending formal appraisals with their manager at least every 12 months, with regular reviews of performance and training objectives.
15. Contributing to public health campaigns (e.g., flu clinics) through advice or direct care.

Lambourn Surgery is located in an area of outstanding natural beauty in a semi-rural setting, serving a population of 6,500 patients. We dispense to 3,000 of our patients. The village is known for its horse racing and is within a short drive of Swindon, Newbury, and junctions 14 and 15 of the M4.

Person Specification
Qualifications:
* Essential: GCSE grades A-C in English and Maths.
