An exciting opportunity has arisen for an experienced Band 6 Community Nurse to take on the role of Lower Limb Coordinator within our Community Nursing Service.
The post holder will lead on the assessment, management, and treatment of patients with lower limb conditions, particularly leg ulcers, delivering high-quality, evidence-based care. A defined caseload will be managed, working autonomously and collaboratively within the wider multidisciplinary team.
This role offers a balance of clinic-based work and community visits, enabling the delivery of holistic, patient-centred care in structured and home environments. The post holder will also support and develop staff, contribute to service improvement, and ensure best practice in lower limb care.
The post holder must be a Band 6 community nurse registered with the NMC, and will be responsible for patients allocated to the lower limb coordinator for leg ulcer management and treatment. Professional and legal accountability applies.
Main duties of the job
* Holistic assessment, planning, implementation and evaluation of care for patients with leg ulcers
* Monitoring and reviewing patient treatment plans
* Prescribing: nurse prescribing and non-medical prescribing
* First contact assessment, care and referral
* Continuing care, rehabilitation and delivery of National Service Frameworks
* Providing public health and health-promotion programmes to improve health and reduce inequalities
* Compression therapy
* Caseload management
* Maintaining contemporaneous written and computerised records
* Leg ulcer audits
* Decontamination audits
Probationary period
This post will be subject to a probationary period, unless the applicant is an internal candidate already in or past the probationary period.
High Cost Area Supplement – Outer London
Payment for the High Cost Area Supplement of 15% of the basic salary is provided (with a minimum of £4,714 to a maximum of £5,941 per annum pro rata for part-time).
