Overview
Are you passionate about making a real difference to the lives of patients and families living with heart failure? We are looking for a highly motivated and compassionate Clinical Nurse Specialist to join our consultant-led Heart Failure and Device Remote Monitoring Service at Liverpool Heart and Chest Hospital. This is an exciting opportunity to be part of a dynamic and innovative service at the forefront of cardiac care. You will deliver expert, holistic care to heart failure patients using remote monitoring systems, supporting earlier intervention and better outcomes for patients across our region. You will play a vital role in providing specialist nursing input across inpatient and outpatient settings, supporting patients through the optimisation of evidence-based therapies and holistic care planning, supporting patients with implantable cardioverter defibrillators (ICDs) and other cardiac devices, providing post-implant education, and integrated deviceHF management.
Responsibilities
* Manage your own caseload of heart failure patients
* Conduct comprehensive assessments, plan and implement personalised care
* Use your Non-Medical Prescribing qualification to optimise therapy
* Deliver care across inpatient and outpatient settings in the trust, including remote monitoring follow-up
* Provide structured education for ICD patients
* Prevent unnecessary hospital admissions, triage and act on HF risk alerts, undertaking patient calls and initiating timely interventions and enhancing palliative and long-term condition care (including ICD deactivation discussions when appropriate)
* Work closely with the heart failure and device consultants in the hospital to ensure all patients have a clinical management plan
* Promote and liaise with the multi-disciplinary team to improve patient and carer quality of life and satisfaction
* Ensure eligible patients with heart failure and post-ICD implant are referred to cardiac rehabilitation and liaise with the Cardiac Rehabilitation Lead and MDTs to support programme delivery
* Address psychosocial-economic needs of patients and carers and refer to other services as appropriate
* Maintain advanced clinical skills to analyse history, symptoms, and findings for diagnosis, planning and treatment
* Communicate complex information empathetically, including end-of-life discussions where appropriate
Qualifications
* Registered Nurse with relevant postgraduate qualifications
* Minimum three years experience in a cardiology setting
* Qualified Non-Medical Prescriber (essential)
* Excellent communication, interpersonal and clinical decision-making skills
* Knowledge or experience of cardiac devices/CIED care is desirable (training and support will be provided)
* A strong commitment to innovation and patient-centred care
* A car driver with access to a vehicle is desirable
