Asa General Practitioner the post holder will be an autonomous professional andan integral member of the practice team.The post holder will manage a caseload and deal with a wide range ofhealth needs in a primary care setting, ensuring the highest standards ofcare for all registered and temporary patients.

Dutiesand Responsibilities

Clinical Responsibilities

In accordance with the practicetimetable, as agreed, the post holder will make him/herself available toundertake a variety of duties including surgery consultations, telephoneconsultations and queries, triage, visiting patients at home, checking and signingrepeat prescriptions and dealing with queries, paperwork and correspondencein a timely fashion. The post holderwill be expected to take part in on-call or duty doctor responsibilitieson a rota basis, normally once per week.

Making professional, autonomousdecisions in relation to presenting problems, whether self-referred orreferred from other healthcare workers within the organisation.

Assessing the health care needs ofpatients with undifferentiated and undiagnosed problems.

Screening patients for disease riskfactors and early signs of illness.

In consultation with patients and inline with current practice disease management protocols, developing careplans for health.

Providing counselling and healtheducation.

Admitting or discharging patients toand from the caseload and referring to other care providers as appropriate.

Recording clear and contemporaneousconsultation notes to agreed standards.

Record all relevant data to aid thepractice in meeting its performance targets.

Compiling and issuing computergenerated acute and repeat prescriptions (avoiding hand written prescriptionswhenever possible).

Prescribing in accordance with thepractice prescribing formulary (or generically) whenever this is clinicallyappropriate.

In general the post holder will beexpected to undertake all the normal duties and responsibilities associatedwith a GP working within primary care.

Administrativeand Other Responsibilities

To manage the normal administrative workloadgenerated by patient contact such as referrals by e-referrals, letters,medical certificates, queries from other health professionals, third partyreports etc.

Practice administration which is done on ashared/rota basis eg repeat prescriptions, clinical correspondence, results.

Awareness of and compliancewith all relevant practice policies/guidelines/protocols eg prescribing.

A commitment to life long learning andaudit to ensure evidence based best practice.

Contributing toevaluation/audit and clinical standard setting within the organisation.

Contributing to thedevelopment of computer based patient records.

Contributing to the summarising ofpatient records and read coding patient data.

Attending training and eventsorganised by the practice or other agencies, where appropriate.

Inthe course of seeking treatment, patients entrust us with, or allow us togather, sensitive information in relation to their health and othermatters. They do so in confidence andhave the right to expect that staff will respect their privacy and actappropriately.

Inthe performance of their duties outlined in this job description, the postholder may have access to confidential information relating to patients andtheir carers, practice staff and other health care workers. They may also have access to informationrelating to the practice as a business organisation. All such information from any source is tobe regarded as strictly confidential.

Informationrelating to patients, carers, colleagues, other health care workers or thebusiness of the practice may only be divulged to authorised persons inaccordance with the practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data.

Supportthe confidentiality and Data Security and Protection agenda in line withPractice policies and current legislation.

Health and Safety

Ensureall accidents and dangerous incidents are investigated, recorded, reportedand where necessary any follow up action taken.

Inline with Practice policies ensure teams adhere to their individualresponsibilities for infection control and health and safety, using a systemof observation, audit and check, hazard identification, questioning,reporting and risk management.

Ensureall health and safety and infection control mandatory training is up to date.

Thepost holder will strive to maintain quality within the practice, and will:-
