Following the national release of the ARRS GP information & funding, Derbyshire Dales PCN is looking to recruit a newly qualified GP (within 2 years of CCT date and not having held a substantive post before) to join our Home Visiting Service on a permanent contract.
You will be required to work 5/6 sessions across the areas supported by DDPCN HVS.
DDPCN collaborates with seven GP Practices in our locality, providing a Monday to Friday home visiting service:
* Evelyn Medical Centre, Hope
* Eyam Surgery
* Baslow Health Centre
* Peak and Dales Medical Partnership (Bakewell & Tideswell)
* Darley Dale Medical Centre
* Imperial Road Group Surgery
* Lime Grove Medical Centre
We are a supportive PCN and will facilitate your work across different surgeries with a lead GP/Clinical Director as a mentor.
This role offers diverse experience and an opportunity to work across north and central Derbyshire Dales. There may be opportunities for short-term secondments within our PCN Family to enhance your skills in a GP setting, including the possibility of expanding beyond home visits. This is under review and can be discussed during interview.
Main duties of the job
To work as an autonomous GP, assessing, diagnosing, and treating patients requiring urgent or proactive home visits, in collaboration with healthcare professionals.
To provide expert advice, maintain clinical excellence, and participate in staff education and training.
To be professionally accountable for all clinical practice elements, ensuring patient safety and care quality in accordance with GMC standards.
The role requires independent travel across Derbyshire Dales and flexible working to meet business needs.

Clinical responsibilities
Participate in the Home Visiting Service rota, undertaking consultations (including telephone), home visits, prescription management, and administrative tasks efficiently.
Make autonomous clinical decisions, assess patient needs, and develop care plans in line with current protocols.
Provide health education and counselling, manage patient admissions/discharges, and refer to other providers as appropriate.
Maintain accurate, contemporaneous consultation records, and contribute to audit activities.
Manage prescriptions, prescribing appropriately per formulary or generically.
Respond to emergencies by visiting patients if clinically indicated, and share home visiting duties fairly.
Proficiency in EMIS and SystmOne platforms is required.

Qualifications
Essential
* Experience in primary care
* CPD and clinical audit experience
* Within 2 years of CCT, not previously employed substantively at a GP practice
* GMC registration, UK practice eligibility, full indemnity, and validation
* Full UK driving license
Desirable
* Experience with medicines management and ICB initiatives
