Our community cardiology team are seeking an exercise professional to join our cardiac rehab service. This is a job share post so you will be required to work Monday to Wednesday.
We are a team of nurses, physio, associate and assistant staff. We cover all of North East Essex so access to a car and a driving licence is essential.
We provide clinic based, class based and home based care, with the aim of supporting people to have quality of life, rehabilitation, and supporting how to manage their cardiac condition.
* To assist in the delivery of patient care, maximizing the cost-effective use of professional time, both clinically and non-clinically
* To assess patients, provide programme of advice and rehabilitation activities to patients with acute or chronic needs within the competency framework developed by the Physiotherapist or Nurse.
* To organise and run exercise classes in the community and on virtual platforms with the registered staff, maintaining high professional standards.
* Able to work independently without direct supervision, but under instruction from the qualified staff, reporting back patient progress and informing the qualified clinicians of any problems.
* To independently progress treatments of delegated patients with access to guidance and determine when the patients have met their discharge criteria.
* To participate in audit and data collection including coordinating the uploading of data to the national audit portal.
