Job Description:
Job Summary:
The Team Leader is responsible for supporting individuals' practical, physical, and emotional needs within the service. They will design support programs and coordinate their delivery by Support Workers.
Key Accountabilities:
* Plan and coordinate the safe transition of new individuals into the service and community.
* Liaise with families, multidisciplinary teams, community groups, and voluntary bodies to promote the needs of the people supported.
* Develop and implement support programs to enhance independence.
* Conduct ongoing assessments and reviews.
* Support the physical, emotional, and personal needs, respecting dignity and independence.
* Manage medication according to policies and legislation.
* Provide leadership and supervision to Support Workers.
* Participate in training and personal development.
* Maintain accurate records and reports.
* Assist with regulatory compliance.
Requirements of the Role:
* Participate in appraisals and staff reviews.
* Model company values and behaviors.
* Engage in supervision sessions.
* Perform duties according to policies and regulations.
* Work flexible hours, including sleep-ins with additional pay.
Qualifications and Experience:
At least 2 years’ experience in health and social care or relevant qualifications such as Social Work, Nursing, or Level 5 Leadership and Management. Alternatively, 1 year paid experience in HSC with a Psychology Degree. Candidates should be prepared to complete Level 2 Safe and Effective Care Practice Diploma or be registered with NISCC/NMC or an Allied Health Professional. A valid driving license and access to a vehicle are required.
