This is an excellent and rare opportunity for a highly motivated and enthusiastic nurses to join the specialist palliative care Team at East Lancashire Hospitals Trust
You will support the delivery of a specialist palliative care nursing, seven-day service, across the hospital and community settings.
Generalist palliative care experience is needed for this role, and must be demonstrated in your application form.
Please be advised you may be required to work across both the hospital and community settings. subject to your development to and to meet service demand.
Staff will be expected to work flexibly over seven days, including weekend and Bank holidays.
Main duties of the job
The post holder will contribute to the provision of a co-ordinated and comprehensive specialist palliative care service for patients, families and carers across ELHT.
Have responsibility for a case load of patients with Specialist Palliative Care Nursing needs.
Assess patients, formulate management plans and implement care by providing specialist nursing advice on complex pain and symptom management and maintain associated records.
Act as a specialist resource for advice on complex pain and symptom management to health care professionals across ELHT.

You will be working in a team consisting of Clinical Nurse Specialists, the Lead Nurse and Assistant Director of Nursing, Consultants in Palliative Medicine, an Advanced Clinical Practitioner and the administrative team, and working closely with the End of life and Bereavement team.

Detailed job description and main responsibilities
Provide evidence based specialist palliative care, advice and support to patients with cancer or other life limiting illness, their families and carers.
Manage a case load of patients with specialist palliative care needs, alongside colleagues, in a holistic manner assessing, developing and implementing programmes of care and managing their complex physical, psychosocial, emotional and spiritual needs.
Organise and manage own workload to ensure the delivery of care, to meet patient needs.
1. Ensure patients are referred appropriately to key health care workers when specialist palliative care intervention is complete
2. Work collaboratively with multi-professionals teams across the health economy
3. Maintain accurate and effective electronic patient records, ensure they are kept updated and communicated to relevant members of the multi-disciplinary team.
4. Collect and collate statistics as required in accordance with professional, national and organisational requirements.
5. Act as a core member of the palliative care MDT ensuring that all new and complex patients with specialist palliative care needs are assessed and discussed as appropriate.
6. Participate where appropriate to non-clinical aspects of the clinical nurse specialist role including audit, education and research.
7. Provide specialist palliative care advice, education and training to all members of the multi-professional team across all care settings as the service requires.
8. Facilitate open and effective communication within the multi-professional team.
9. Maintain good relationships and communication with all disciplines within the trust and external partners.
10. Ensure that resources are used efficiently and effectively and report any causes for concern to the appropriate manager.
11. Establish, utilise and maintain advanced communication skills in a variety of challenging situations including patients, carers, families and the multi-professional team.
12. To be empathic and reassuring when communicating highly sensitive information and advice to patients, families, carers and colleagues.
13. To work within the competencies of V300 nurse independent prescriber if qualified
o Be involved and contribute to clinical protocols and developments for the specialist palliative care service.
o Embrace and support line management and the department in making service improvements effective in the workplace.
o Ensure compliance with organisational policies and procedures and national, legal and professional standards.
o Be an active member of appropriate groups representing needs of patients and appropriate staff.
o Contribute to the development and delivery of a seven day service working flexibly over the seven days to meet the needs of the service.
o Maintain accurate up to date information relevant to service compiling statistical evidence as required..
o Contribute to management of complaints and incidents.
o Work within palliative care clinical governance framework.
o Contribute to the achievement of ELHTs objectives and targets for Specialist Palliative and End of Life Care

* Registered adult nurse with on the NMC register
* Demonstrable post registration experience in palliative care
* On a relevant first degree pathway
* Demonstable teaching qualification or equivelent experience
* Communication skills training/sage and Thyme
* Flexible to work 7 days and access to a car for work purposes
* Evidence of recent study or personal development
Desirable criteria
* Advanced communications skills training
* V300 prescribing.
* Understanding of palliative and end of life care local and national drivers.
