RN Care Coordinator - Infectious Disease Clinic
The RN Care Coordinator is responsible for day-to-day management of patient care from initial contact through recurring visits. Ensures patient care services are timely and provided at the appropriate level of care. The primary goal of this role is to support and engage in activities that result in effective patient care and efficient use of resources along the care delivery continuum.
Responsibilities
* Manage patient care from initial contact through recurring visits.
* Coordinate care to ensure timely services and appropriate levels of care.
* Support and engage in activities that lead to effective patient care and resource utilization.
Minimum Education
* Graduate of an accredited school of nursing; BSN preferred.
Minimum Work Experience
* 2 years related clinical RN experience per specialty area.
* Nursing charge or lead experience strongly preferred.
* Experience educating a diverse population on healthcare issues highly desirable.
Required Licenses/Certifications
* Current Vermont RN license.
* BLS Certification through American Heart Association.
* Valid Driver's License (if conducting home visits).
* Department-specific Certifications may apply.

Pay Range
$36.15 - $61.15
