Critical Care Outreach Practitioner CNS, Band 7

Permanent 37.5 hours per week

We are looking for an enthusiastic and highly motivated nurse with critical care/ acute clinical and leadership experience to join our Critical Care Outreach team (CCOT) as we expand our service to provide 24/7 cross site cover.

The successful applicant will join a supportive and dynamic team, leading on delivering high standards of care to acutely unwell patients across our site. CCOT teams are a nurse led service supported by the critical care consultant team. You will have the opportunity to work both as a team and independently, identifying patients at risk of clinical deterioration or that are deteriorating have their needs prioritised with appropriate assessment, intervention and on-ongoing management. Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across site.

We will provide learning opportunities to develop clinical skills within the CNS role.

Main duties of the job

1. Collaboratively work as an integral member of the Multidisciplinary Team to providing safe and effective care in ward areas.
2. Recognise and respond to acutely ill and deteriorating patients ensuring timely interventions by the most appropriate member of the healthcare team particularly out of hours.
3. Respond to clinical emergencies as they arise.
4. Teach and assess clinical skills in practice as the opportunity arises.
5. Act as a professional role model for acute care.
6. Audit patient outcomes and service delivery and work collaboratively with other key personnel to introduce change for service improvement
