The Transfer of Care Hub (ToCH) is a service that implements an expert complex multi‑disciplinary and inter‑disciplinary discharge team, working seamlessly across partner organisations, both health and social care. The ToCH proactively pulls and case‑manages the needs of people with complex discharge needs on pathways 1‑3 and progresses them safely to transfer/discharge via an appropriate pathway.
The Transfer of Care Hub Lead Therapist (Band 7) is a senior clinical leadership role responsible for delivering high‑quality, timely, and patient‑centred discharge planning within a fast‑paced acute setting. Acting as the therapy lead within the multidisciplinary ToCH, the post holder provides specialist clinical expertise, leads the therapy contribution to Discharge to Assess (D2A) pathways, and supports safe, effective patient flow across health and social care systems.
The role involves close collaboration with internal teams and external partners, providing leadership, supervision and professional guidance to the MDT, while contributing to service development, governance and performance improvement. The post holder will play a key role in reducing delays, improving patient outcomes and ensuring discharges are coordinated efficiently and in line with national and local priorities.
Main duties of the job
* Lead the therapy contribution within the Transfer of Care Hub, supporting safe, timely and effective patient discharge.
* Provide highly specialist clinical assessment, decision‑making and advice for patients with complex needs.
* Act as the first point of escalation for therapy‑related discharge issues, resolving barriers and supporting patient flow.
* Lead and coordinate the Discharge to Assess (D2A) pathway in line with national guidance.
* Work collaboratively with multidisciplinary teams and system partners across health and social care.
* Provide clinical leadership, supervision, training and support to therapy staff and the wider team.
* Contribute to service development, clinical governance, audit and quality improvement activities.
* Support performance management by monitoring discharge metrics and addressing delays in care pathways.
* Maintain a clinical caseload and work autonomously within professional scope.
Person Specification
Qualifications
* Qualified to degree level or equivalent; evidence of post‑registration qualifications/professional development in related health field.
* Therapy‑based Degree/Diploma.
* Membership of relevant professional body.
* Registered with a Health Professional Council.
* Training and/or qualification relating to clinical supervision.
* Independent non‑medical prescribing qualification.
* Post‑graduate teaching qualification.
Knowledge and Experience
* Minimum of five years’ experience post‑qualification.
* Broad range of clinical experience.
* Experience in supervision and appraisal of staff.
* Multi‑disciplinary team working.
* Demonstrable experience of providing teaching/training.
* Experience of Discharge to Assess Pathway.
* Completed audit projects.
* Service/practice innovation.
* Demonstrable interest in continuing to develop knowledge and skills in area specialism.
* Evidence of continuing professional development.
* Ability to manage a complex caseload.
* Ability to drive high‑level multi‑disciplinary practice to maximum potential.
