To work under the management of the Clinical Pharmacists in the North Neighbourhood to deliver a comprehensive medicines management to ensure a safe and efficient service is maintained, delivering a range of medicines management activities including medicines reconciliation and repeat prescription requests To liaise with pharmacy staff in all sectors and other healthcare professionals to ensure a patient orientated medicines management service is provided, conducting medicines reviews for patients identified by the wider multidisciplinary team or from other medicines management initiatives. To work closely with the Clinical Pharmacists to adopt a team approach to medicines management, acting as a key member of the multidisciplinary team and the wider pharmacy team, providing medicines management advice to healthcare professionals, patients and carers. To work with the Clinical Pharmacists to develop and undertake medicines management audits, to support discussion within the practice and agree subsequent actions. To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within Northumbria Primary Care.
