About the Role
As a Provider Support Assessor, you’ll play a vital role in assessing people’s care and support needs and developing reablement programmes for adults, including people leaving hospital and people with complex and changing needs. You’ll work closely with individuals, carers, Home First - Community Service colleagues and professionals to promote independence, choice, and social inclusion.
What You’ll Do
5. Carry out assessments and develop tailored support plans.
6. Monitor and review reablement programmes.
7. Work collaboratively with internal and multi-agency colleagues.
8. Maintain accurate records and contribute to service planning.
9. Support service users to access community resources and build life skills.
What We’re Looking For
10. Experience in working with people, possibly in adult social care, particularly with older people or those with complex needs
11. Strong communication and teamwork skills
12. Knowledge of CQC standards
13. A recognised Health or Social Care qualification (NVQ/QCF Level 3 or equivalent) or willingness to work towards one
14. A commitment to equality, dignity in care, and continuous personal development
15. Excellent computer skills
16. Own car driver
