Occupational Therapist – Community OT (Pathway 1 Reablement, Adults) – Wolverhampton
Contract: 12 weeks | Full-time | 37 hours per week | Locum
A local authority in Wolverhampton is seeking an experienced Occupational Therapist to join their Community OT Team, working within Pathway 1 Reablement for adults.
This is a short-term but impactful role supporting service users to regain independence and reduce long-term care needs through targeted, strengths-based reablement interventions.
Main Duties:
Complete community-based OT assessments within an adult reablement (Pathway 1) service
Deliver short-term, goal-focused interventions to maximise independence at home
Assess functional ability and implement appropriate equipment, adaptations, and support strategies
Support hospital discharge and admission avoidance where required
Work collaboratively with reablement staff, social care, and multidisciplinary teams
Manage a community caseload within a fast-paced service
Promote strengths-based practice and independence-focused outcomes
Ensure timely and accurate case recording in line with local authority proceduresRequirements:
HCPC Registered Occupational Therapist
Experience within adult social care or community OT services
Strong reablement experience (Pathway 1 desirable)
Confident in equipment provision and functional assessments
Ability to manage a fast-paced caseload and deliver time-limited interventions
