Join our new Assertive Outreach Team as a Support, Time & Recovery (STR) Worker (Band 4) and help shape a service that makes a real difference for adults with severe and enduring mental health needs.

This is an exciting opportunity to work with individuals who find it difficult to engage with traditional services, often facing challenges such as substance misuse, homelessness, or social isolation.

You'll be part of a dynamic, multi-disciplinary team, providing intensive, flexible, and person-centred support that empowers service users to achieve greater stability, independence, and quality of life. You will be required to travel independently around the county and will need to hold a full UK driving licence* and have the use of a vehicle. Unfortunately, public transport will not provide enough reliable transportation to enable you to meet the needs of the service. * As an STR Worker in the Assertive Outreach Team, you will directly engage with adults experiencing complex mental health conditions who struggle to access or engage with services. Working under the supervision of registered practitioners, you'll provide practical and emotional support, help implement care and treatment plans, and promote recovery and inclusion within the community.

* Your role will involve building trust, assessing immediate needs and risks, and co-ordinating care to help service users sustain housing, improve quality of life, and move towards greater independence.

* You'll support access to activities and appointments, contribute to risk management, and work collaboratively with colleagues and partner agencies. Commitment to safeguarding, equality, and continuous professional development is essential.

Detailed job description and main responsibilities

1. To support the care co-ordinator and other staff in the team to manage patient care, including practical interventions and supporting patients when there is a crisis, under supervision of a registered clinician.

2. Under the supervision and direction of registered professionals develop therapeutic relationships with patients and carers within appropriate and transparent boundaries.

3. To engage service users effectively with the agreed care plan and to ensure access to appropriate services are obtained on a needs led basis.

4. To be the second clinician accompanying clinical staff to assess or review patients off base where needed.

5. To participate in group work where appropriate.

6. To deliver medication and facilitate wellbeing by supporting the consultant psychiatrist, registered professionals, outpatient appointments including physical health appointments.

7. Plan and organise own work diaries in an effective and efficient way, under supervision of registered practitioners.

8. Develop an alliance based upon collaboration and mutual respect and within appropriate and transparent boundaries.

9. Support patients to gain access to resources to include benefits and welfare rights, using internal and external expertise as required.

10. Where appropriate help to identify early signs of relapse by monitoring the patients progress, level of functioning and mental state and alert the appropriate staff involved in the individual's care
