We're looking for a dedicated and compassionate Young Persons Advisor to join our 15+ Team within Social Care and Housing. This role is central to supporting care-experienced young people as they transition to independent adulthood. You will provide practical advice, emotional support, and advocacy to help young people navigate housing, education, employment, and health services. Working closely with social workers and other professionals, you will co-develop and review pathway plans that reflect each young person's strengths, aspirations, and needs. You will also act as a key worker for care leavers, ensuring their voices are heard and their progress is tracked, while responding flexibly to crisis situations and promoting independence at every step.
