We are looking to recruit a Care Co-ordinator post within our Adults Safeguarding Team, to enable us to improve our services to Adults and Older People within the Wakefield District.
This is an opportunity to be part of a new and exciting challenge within the area of Adults Operations.
Working as part of an Adult Safeguarding Team you will be a:
1. A Champion within the Adult Safeguarding Team within the organisation
2. Provide specialist advice and coordination
3. Respond to concerns and make enquiries
4. Work with the person subject to abuse
5. Co-ordinate who will do what $ú e.g. criminal or disciplinary investigations
6. Work closely with CQC
What will be expected from me
7. You will play an important role in helping vulnerable people live as independently as possible.
8. Taking responsibility for assessments and re-assessments, identifying where vulnerable adults may be at risk.
9. You will be pro-active in establishing eligibility, signposting to relevant services, assessing and determining the level of need to create outcome plans that reflect a person centered approach.
Essential requirements
10. Although it is not essential for you to have a social work degree qualification for these posts, it is essential you have knowledge of care management and understanding of the Care Act .
11. It is also important that you are an organised individual who has current knowledge of all relevant legislation.
