Job Overview
The primary role of this specialist post is to lead on the management and provision of a comprehensive, high quality pharmacy service to the Neurology, Neurosurgery and Stroke patients within the organisation by working in partnership with the Senior Pharmacist for Stroke Services and Senior Pharmacist for Neurosciences. The post holder will share an overarching role to organise and deliver pharmacy related care to all these patients Trust wide and support the directorate(s) as needed.

Responsibilities
* Lead, provide and develop a clinical pharmacy service for stroke and neurology patients at UHCW NHS Trust in partnership with the Senior Pharmacist for Stroke Services and Senior Pharmacist for Neurosciences.
* Work closely with the pharmacy neurosciences team, along with other relevant surgical and medical pharmacists, supporting and covering, while developing services together.
* Be responsible for all aspects of medicines optimisation including providing a cost-effective clinical pharmacy service which is patient and drug orientated through effective supply of medicines, prescription monitoring and prescribing which ensures the patient receives safe, effective and timely drug therapy.
* Work with other Healthcare Providers and centres especially with regards to specialist areas where UHCW may be either a hub, spoke or regional centre.
* Lead on the provision of education to healthcare professionals in the pharmaceutical and medicines management and optimisation aspects of their area(s).
* Be responsible for the development of policies, guidelines, audits, reports (both financial and clinical) and research relating to the use of medicines within their area (including high cost drugs).
* To lead, provide and develop comprehensive clinical pharmacy service for patients at UHCW NHS Trust attending multidisciplinary team meetings and speciality forums as required.
* As a key member of the multidisciplinary team provide strategic guidance on clinical pharmacy issues and provide professional pharmacy leadership.
* To practice as an expert practitioner, providing advice to patients, healthcare professionals and other agencies relating to the pharmaceutical aspects of stroke, neurology and neurosurgery.
* Proactively influence safe and cost-effective medicines use with other senior clinical staff within stroke. This will include new drug and device evaluations, financial reporting and development of policies.
* Identify areas where improvements in both quality and cost effectiveness of pharmaceutical care and prescribing may be encouraged.
* Work with senior staff in your areas to review and develop policies and procedures to ensure safe and effective handling of medications. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and Health Care Commission Requirements.
* Undertake risk management and develop audits/policies relating to supply, storage, prescribing and administration of medicines and other pharmaceuticals.
* Advise on the appropriate prescribing and monitoring of anticoagulation for stroke patients, including the counselling of these medications to stroke patients.
* Lead, develop and deliver education and training in the pharmaceutical aspects of stroke and neurosciences to other healthcare professionals. This will include trainee pharmacists and diploma students.
* To encourage and cultivate an environment of clinical and practice-based research and audit. To contribute to current and future research and audit projects associated with the use of drugs within your specialist area(s).
