Key Responsibilities: Conduct comprehensive and holistic assessments of adults with long-term conditions and their carers in the home environment Manage and organise your own caseload, ensuring timely assessments and reviews Develop a strong understanding of the role of equipment, minor/major adaptations, and housing solutions in promoting independence Provide input to OT assessment clinics across the county Build competence in moving and handling techniques, including single-handed care Contribute to OT triage, gaining insight into referral management and innovative risk-reduction strategies Participate in service development, audits, evidence-based practice, and raising the profile of community OT Engage in regular supervision and professional development, identifying and addressing training needs Supervise and mentor appropriate staff across the integrated service Collaborate with health, housing, and social care colleagues to ensure effective partnership working Deliver person-centred rehabilitation and support individualised goals Undertake additional duties as deemed appropriate by the Team Manager Benefit: Hybrid working available Qualifications and Experience: Degree/Diploma in Occupational Therapy HCPC registration Experience working with adults with long-term conditions in a community setting Strong understanding of equipment provision, adaptations, and housing legislation Skilled in manual handling and promoting functional independence Excellent communication and organisational skills Commitment to lifelong learning and service improvement Referral Bonus: Refer a friend and receive a bonus – contact me for details!
