Rehabilitation Assistant

We are seeking a dedicated and compassionate Rehabilitation Assistant to join our Community Rehabilitation Team on the Isle of Wight.
In this role, you will support the delivery of high quality, multidisciplinary rehabilitation care across both NHS commissioned Community Rehab beds and patients' own homes.
Working under the delegation of Registered Nurses and Therapists, you will help patients regain independence, achieve personal goals, and transition safely back into their community.
This is a 7day service, offering a dynamic and rewarding environment where you will play a vital part in improving patient outcomes.
Main duties of the job
* Washing and dressing
* Meal preparation
* Progression of patients with walking aids (crutches to walking stick for example)
* Transferring from chair to bed etc and monitoring progress with this
* Progression of patients with stair practice
* Visiting patients at home and fitting of certain prescribed equipment
* Bathing assessments post discharge of patients from Community Rehabilitation provider beds

Qualifications
* Care Certificate (or working towards completing the care certificate)
* Functional Maths & English
* Basic office skills/stores ordering
* Attend annual update and complete e-learning modules as required
* Develop own knowledge and skills and provide information to others to help their development
Experience
* Physically capable of performing the role and responsibilities expected of the post holder.
* Prioritise work and work to deadlines.
* Plan tasks on a daily basis dependent on requests made.
* Report back any clinical/ social concerns to a qualified member of staff.
* Evidence of commitment to personal development and training.
* Evidence of commitment to health and safety issues.
