Job Summary Join our team of GPs and ANPs in the New Models of Care programme for the Northamptonshire Rural East & South (NRES) Neighbourhood.
Work within our practices across the neighbourhood to deliver high‑quality, same‑day care to patients.
We’re looking for GPs to join our Neighbourhood Access Service, part of the innovative New Models Programme. You will work autonomously and as part of a multidisciplinary team to provide expert assessment, diagnosis, treatment and evaluation for patients presenting with undifferentiated primary care presentations. In addition to same‑day access work, you will be expected to incorporate a proportion of routine GP appointments into your working day.

Clinical Responsibilities
Provide initial history taking and clinical assessment, diagnosis, treatment and evaluation of care for patients presenting with undifferentiated primary care conditions.
Conduct telephone triage and clinic appointments to reduce demand on core neighbourhood GP practices.
Use advanced skills and expert knowledge to assess physical and psychosocial needs of patients with minor illness and instigate therapeutic treatments based on best available evidence to improve health outcomes.
Make comprehensive, focused assessments and manage care of patients independently and proactively.
Promote patient independence and self‑care where appropriate.
Support and work cohesively with other GPs, ANPs and ACPs, providing oversight and mentorship for more junior colleagues.
Review pathology results to ensure appropriate treatment and care.
Support and lead the practices in the delivery of the Quality and Outcomes Framework and NHS England, ICB, Federation contracts.
Regulatory Responsibilities
Respond to complaints and clinical incidents, significant events as appropriate, ensuring robust systems for improving practice through joint learning.
Maintain accurate and timely clinical records in line with professional and legal standards.
Undertake at least one clinical audit a year, evaluate data, present findings to the practice clinical team and implement changes where appropriate.
Adhere to the employing practice policies and procedures including infection control, health and safety, and safeguarding. Ensure all mandatory training is completed in a timely manner.

Personal and Professional Development
Maintain own personal/professional development plan and portfolio to meet the requirements for revalidation through appraisal of own performance.
Adhere to the annual appraisal process and revalidation requirements.
Work at all times within the GMC Code of Professional Conduct.
Provide mentorship and training of new recruits or students as and when required.
Participate in the practice performance review process.
Qualifications – Essential
CCT qualification

Relevant first level degree
Extensive clinical experience in primary care
Evidence of continuing professional development
Qualifications – Desirable
Relevant Master level degree
Management qualification or experience
Post registration qualification in relevant areas
Teaching qualification
Recent attendance on a leadership skills development programme

In‑depth knowledge base in the management of patients with chronic illness and minor illness

Understanding of clinical governance issues
