About the Role
Sue Ryder invites applications from experienced nurses to join a friendly, well-established team as a Clinical Nurse Specialist in Reading. You will support patients with an end-of-life diagnosis to remain in their homes. This role is primarily telephone triage.
Responsibilities include assessing referrals, performing comprehensive patient assessments, planning, implementing, and evaluating care, identifying potential palliative care emergencies, developing action plans, and ensuring effective communication with patients, families, and other professionals. Additional duties include:
* Using advanced assessment skills to identify reversible symptoms and develop care plans
* Providing holistic, specialist care tailored to patient needs, wishes, and culture
* Establishing effective communication with patients, carers, and professionals, often of a complex nature
About you
We seek a registered nurse with experience in community palliative care or related fields, with evidence of ongoing professional development in this specialty. You should have excellent communication skills and qualifications in advanced clinical assessment, mentorship, and non-medical prescribing, or be willing to undertake training.
