Permanent, Full time, 37 hours per week.
An exciting opportunity has arisen as an Assessment & Review Co-ordinator to work with young adults, supporting them to prepare for adulthood. We are expanding the team and developing the Preparing for Adulthood offer in Rotherham.

About the Team
The Preparing for Adulthood (PFA) Team provides an opportunity to develop skills in working with individuals as they prepare for adulthood. The team works collaboratively with other professionals, organisations/providers, carers and families and is pivotal in shaping and driving the Preparing for Adulthood programme forward. We are keen to develop and improve the lives of Rotherham residents and shape the commissioning of services.
About you

* Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, Making Safeguarding Personal, and Continuing Healthcare Frameworks.
* Root your practice in compassion, values and the understanding of the why of what we do in our profession to promote people’s wellbeing.
* Be confident in working with situations of varying complexity and able to make appropriate decisions quickly to support those in crisis, to ensure people are seen and supported in a timely manner.
* Know how to work with partner agencies to help people find solutions to maximise independence and resilience, enabling people to take positive risks and lead good lives.
