Job overview

The purpose of this post is to support the Patient Flow Coordinator with capacity and
flow on the general adult and older people’s wards.
The post holder will support the Patient Flow Coordinator in ascertaining the trust
position regarding bed management resources.
They will support in providing a comprehensive handover to the Night Managers and
coordinate receipt of a full handover the following day.
The post holder will be responsible for updating and maintaining systems to enable
accurate bed states across the organisation and Greater Manchester.


Main duties of the job

To assist the coordination of admission and discharge activities within adults and
older people acute services.
 Ensure an estimated date of discharge is declared within agreed timescales as
per operational policy.
 Ensure the patients care responsibility lies with PCFT
 To participate and assist in establishing an effective system for the management
of beds to ensure efficient patient flow.
 Respond to all requests for emergency admission, discussing alternatives with
the Patient Flow Coordinator where appropriate.
 Discharges.
 To be actively involved in the capture of audit data in response to service need.
 To liaise with the Patient Flow Coordinators on a daily basis to inform of the
current bad states within the organisation.
 To be involved in coordinating the discharge process.
 Record all OAPs in line with GM and National guidance.

Detailed job description and main responsibilities

 Ensure accurate beds states are maintained in conjunction with the inpatients
wards including, admissions, discharges, patients on leave or AWOL and those
patients on a CTO recall.
 Work with Performance and Information colleagues to ensure timely data capture
and entry to support inpatient dashboard.
 Ensure effective communication with Night managers, Consultants, HTT, MHLT
and CMHT and other stakeholders involved in a patient’s care.
 Coordinate bed management meetings in the localities.
 Contact wards daily to understand plans for any patient movement e.g. leave or
discharge.
 Liaise with external partners regarding patients from out of area or cross border
activity.
 Support Patient Flow Hub with recording of non-contracted activity.
 Support with the arrangements of transport for patients.
 To support all aspects of patient flow.
 Work as part of a team to ensure the best outcome for the patient
 Ensure confidentiality is maintained at all times.
 To be self-motivated and manage own workload.
 To participate in training as required

Education Qualifications


Essential criteria

* Diploma level or equivalent experience instead of Degree level qualification or significant equivalent experience
* Evidence of willingness to undertake further personal development


Desirable criteria

* Clinical qualification e.g QAP


Experience


Essential criteria

* Recent experience of working with individuals who present to mental health service in crisis and have symptoms of mental illness.  Demonstrable experience of working with patients with complex needs.  Experience of working with multidisciplinary teams.  Recent experience of working in adult and older people acute mental health setting.  Recent experience of bed management or discharge coordination across a large organisation.


Knowledge


Essential criteria

* Acute hospital provision  Admission, transfer and discharge procedures.  Understanding of funding process for placements.  Knowledge of risk assessment / management processes. Understanding of current mental health legislation, safeguarding and child protection legislation.  Understanding of S117 processes.  Understanding of confidentiality / data protection issues.  Comprehensive understanding of discharge standards, CCG process and specialist services provision.  Understanding of the capacity management and bed management procedures.  Good knowledge of the Mental Health Act


Skills


Essential criteria

* Excellent communication skills, both written and verbal.  Comprehensive and proficient IT skills.  Ability to form productive relationships with patients, carers, multidisciplinary staff and relevant stakeholders.  Good organisation and prioritisation skills.  Working knowledge of delayed discharge criteria and barriers to discharge.  Effective communication with the wards, consultants, and service managers.  Establish and maintain close working relationships with CMHT, HTT, RAID, Social Services and other secondary acre providers.  Effectively communicate with all grades of staff in relation to patient flow
