ABOUT THE ROLE
We have an exciting opportunity for a qualified Nursing Associate to join our established Nurse Associate team in Nottingham West Primary Care Network (PCN). The teams focus is to work with the GP practices to deliver primary care nursing service to housebound patients and care home residents. On occasions there will be a need to work directly in GP practices. Details of the role can be seen in the job description but include supporting patients with long term conditions and frailty, undertaking clinical tasks such as venepuncture, ECG, foot checks and sample collection. In addition there is a requirement for the service to work flexibly to deliver adult immunisation to the wider PCN population.
We are looking for a Nurse Associate who has experience in primary or community nursing and is enthusiastic about providing high quality care in primary care. The postholder will need to be hard working, highly motivated and have the confidence to work on their own in patients homes. They will also work as part of a small team who are flexible in their approach to fit the changing needs of the service.
As a Nursing Associate, you will be primarily supported clinically by the Senior Nurse Associate, the Practice Nurses and the PICS Lead for PCN nursing.

NMC registration as a Nurse Associate
Nurse associate Foundation Degree
GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent

Promoting health and preventing ill health
Support patients to improve and maintain their mental, physical, behavioural health and wellbeing, demonstrated in such as:
using behaviour change interventions to support health and wellness when undertaking health checks (physical and mental) which may include for example: frailty reviews and referring to other services as needed including signposting patients to personalised care colleagues and local community and voluntary sector services
Long term condition management contribute to the monitoring of patients with, for example, respiratory disease, diabetes, coronary heart disease, renal failure, hypertension
Encourage patients to take part in health screening. The role may require, when appropriately trained, to perform cervical screening tests .
Care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities

Contribute to on-going assessment and recognise when it is necessary to refer patients onto others for reassessment
