We have a great opportunity for an Intermediate Care Community Support Worker on a part-time (25 hours per week), permanent basis.


This role follows a three-week rolling rota, and the attached document provides further details.


About you…

You are an enthusiastic, respectful, caring and reliable person who thrives in a fast-paced environment with experience in health and social care, and comfortable working on a three-week fixed rota. You will bring a flexible and adaptable approach, as well as regular access to a car to support your work. You enjoy being part of a large, collaborative team and will be motivated by making a positive difference to the lives of residents in Bracknell.


About the team…

The Intermediate Care Service is a multi-disciplinary team of health and social care professionals working together to help residents live fuller, more independent lives. We provide fast-paced, responsive support to people in their own homes or residential settings following a hospital stay or crisis in the community. Our service supports individuals for up to six weeks as they recover from illness, surgery or injury, helping them regain confidence and independence on their journey to recovery.

* Work as part of an integrated, multi-disciplinary team.
* Provide and work within a planned programme of rehabilitation, care and support for people who use the service following an assessment of their needs, including personal care, meal preparations, exercises and mobility, cognitive therapy and therapeutic moving and handling.
* To monitor and make observations of general behaviour, abilities and responses of people who use the service; record and report information in relation to the person’s situation and review out comes and risk assessments as the person’s condition changes. Maintain accurate records.
* Undertake statutory and mandatory training, participate in identifying own training needs, attend training programmes and contribute to the induction of new staff within the service. Develop and maintain own competencies in all areas of care, support and rehabilitation.
* To take an active role in health promotion including falls prevention, medication support, importance of activity, good nutrition and healthy living with a view to regaining independence in line with departmental policies and procedures.
