South Tees NHS Foundation Trust are looking for a Homeless Prevention & Discharge Officer, to work across the Trust and with our Local Authority partners.You will be the main Trust contact for all patients who are due to be discharged from hospital and require homelessness support. You will provide timely and appropriate advice and support to vulnerable patients in order to prevent and/or alleviate homelessness and where necessary be responsible for liaising with relevant agencies regarding homelessness applications in accordance with legislation and guidance.As well as case managing patients throughout their discharge journey, you will be responsible for setting up and educating on homelessness and prevention within the Trust and act as an ambassador for this cohort of patients.Speaking with homeless patients who are attending A&E and offering advice or help, or to prevent admission will also be a part of this role.Working with local homeless agencies, charities and social care providers will be key to this position.To support homeless patients during their acute in patient stay.To case manage homeless patients throughout their inpatient journey champion with wider awareness of homelessness within the Trust.Completion of relevant trust and Local Authority documentation Liaise with relevant local authorities to support discharge.Provide Education and support to patients and staffSkillsSouth Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills.
