We have an exciting opportunity for a Band 5 Mental Health Nurse to join our Swale CMHT, located in Sittingbourne Memorial Hospital.
The Clinic Nurse is a key member within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver, and evaluate care. The role involves communicating findings, influencing and driving change, and promoting health, best practices, patient safety, and experience within the clinics.
The post holder will be an experienced registered nurse who, acting within their professional boundaries, provides leadership and ensures the clinical governance framework standards are upheld within the clinics.
You will provide care for patients within the clinics from initial assessment, administration of prescribed medication, monitoring health and well-being, including physical health interventions, and evaluating care.
You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. Critical thinking in clinical decision-making and collaboration with the general practice team are essential to meet patient needs and support policy and procedure delivery. Nurse leadership may be required, and NMC requirements for advanced practice must be met.
The role also involves supporting patients at home with their mental and physical health needs.
