Advanced Practitioner – Social Care Screening Team
Salary: £42,839 - £47,181
Location: Grantham Police Station
This role will be based within the Grantham Safeguarding Hub, working in close collaboration with the police. The successful candidate will play a key role in shaping and developing a triage model for contact screening, ensuring effective and timely responses to contacts. Within this role you will work as part of a multi‑disciplinary team in Lincolnshire Partnership hub, and facilitate the sharing of information between partner agencies to enable effective safeguarding practices.
The role is based at Grantham Police Station and requires MV (management vetting) level of clearance.
Responsibilities:
* Work as part of a multi‑disciplinary team to make decisions at the front door and determine the right level of support required for a child or family when concerns arise about their safety or well‑being.
* Screen contacts about children and young people and hold consultations to advise and guide partner agencies.
* Provide strong peer support, formal and informal supervision, and participate in group learning and team meetings.
Qualifications:
* A recognised social work qualification and Social Work England registration.
* Significant social work experience and in-depth knowledge of relevant legislation.
* Knowledge of Social Care Intervention thresholds.
* A firm belief in the importance of social work values and practice.
* A strengths‑based and creative, innovative approach to problem solving.
* An excellent communicator who builds strong working relationships with colleagues, families and external agencies.
