Overview
Central and North West London NHS Foundation Trust, Health and Justice Services, Children and Young people service line provides high quality integrated and trauma‑informed service for young people within youth justice settings. We are looking to recruit a highly motivated, innovative and enthusiastic Primary Care Service Lead to join our team at HMP High Down. As part of the Prisons Health Care Team you will look after the health needs of patients residing in secure environments and have a real impact on promoting health and well‑being. Our services operate 7‑day a week and are managed to the highest standards.
Responsibilities
* Overall responsibility for line‑management, clinical, operational and performance functions, ensuring effective resource utilisation, financial impact awareness and delivery of evidence‑based patient care.
* Provide clear, visible leadership, vision and clinical expertise to enhance the service user experience.
* Implement contracted service development and improvements, day‑to‑day service clinical performance, workforce and budget management, professional clinical leadership and clinical governance.
* Monitor and report on all relevant contractual targets including the DoH Prison Health Performance & Quality Indicators.
* Act as the senior nurse manager contact for primary care on all managerial and clinical matters.
* Attend Trust and Prison‑led meetings as required, enabling effective integration of primary care services with all other aspects of the prison health service.
* Operate flexibly within a challenging healthcare environment involving multiple providers, acting as an ambassador for Healthcare & CNWL.
* Support the team in the creation of effective pathways of care for patients served.
* Deputise for the Head of Healthcare and oversee peer healthcare service manager areas as required.
* Work within a multi‑disciplinary service team approach.
* Deliver effective leadership and direction to all staff within the Primary Care Team.
* Ensure the service is cost‑effective, well‑organised, evidence‑based and service‑user focussed.
* Ensure all targets and performance indicators are met; communicate at risk situations to the Regional Operations Manager and Business and Performance Lead.
* Implement service changes with measurable outcomes and agreed timescales.
* Advance innovative ideas and strategies, providing vision and inspiration to enable staff to deliver care more effectively.
* Provide clinical leadership to multidisciplinary teams across the wider healthcare service.
* Offer specialist primary care nursing advice to GPs and other prison colleagues.
* Realise day‑to‑day practice within a Clinical Governance framework, including setting agendas, incident and complaint management.
* Manage the clinical care of patients in in‑patients, segregation, wings/houseblocks and other prison areas.
* Conduct death‑in‑custody reviews, develop action plans, and audit clinical risk management and best practice implementation.
* Develop nursing practice in primary care and chronic disease management.
* Ensure the clinical environment meets acceptable cleanliness, safety and infection control standards.
* Act as a suitable role model for your grade of employment.
* Complete all relevant documentation in accordance with Nursing and Midwifery Council and CNWL Trust policy.
* Maintain Caldicott and Data Protection Act standards for patient confidentiality at all times.
* Represent the prison healthcare service and CNWL Health & Justice Service at appropriate meetings.
* Comply with the Modern Matrons Charter and ensure services adhere to NMC, NICE, CNWL and prison standards.
* Establish and maintain a professional practice and accountability culture.
* Ensure compliance with Care Quality Commission Provider Compliance Assessments for quality and safety.
* Guarantee that working conditions and resource use satisfy current legislation and establishment guidelines.
* Promote a culture focused on improving outcomes across the care pathway for clients.
* Partner with prison and other healthcare providers to establish clear operational policies, procedures and protocols.
* Take active part in ACCT process and other risk management processes.
* Ensure service users’ health needs are met through equity, fairness, respect and entitlement.
* Line‑manage Primary Care Clinical Team leads and other nurses, providing monthly supervision and annual appraisal in line with Trust policy.
* Ensure the entire primary care workforce, including healthcare assistants and administrators, receive monthly supervision and an annual appraisal.
* Lead recruitment of vacant clinical posts in primary care as requested by the Head of Healthcare.
* Ensure all primary care nursing staff maintain professional registration and pursue development, including mandatory training and special interests (e.g., phlebotomy, wound management).
* Lead the implementation of care quality standards, policy development and improvement programmes across the service.
* Provide cover for the Head of Healthcare as required.
