JOB OVERVIEW

We have a fantastic new job opportunity for a Senior Therapy Practitioner / Health and Social Care Coordinator with experience of working with adults at risk in an adult social work or health environment, safeguarding procedures and good supervisory skills.

Working as a Senior Therapy Practitioner / Health and Social Care Coordinator your role will focus on the development of the integrated neighbourhood team by promoting positive relationships and partnerships with key agencies and professionals working with residents and those accessing primary care in the area.

As the Senior Therapy Practitioner / Health and Social Care Coordinator your aim will be to promote social inclusion and support people with long term health and social care needs by collaborating with community organisations, as well as providing up-to-date information and advice to ensure that people can connect to their community and access support for themselves.

DUTIES

Your duties as the Senior Therapy Practitioner / Health and Social Care Coordinator include:

1. Manage a caseload of clients who have experienced common mental, physical and social health problems or long-term conditions
2. Lead on managing Integrated Care Coordinators and Community Inclusion Workers
3. Develop the integrated neighbourhood team model, focused on bringing together professionals from health, social care and the voluntary sector in one core team to support people to receive the help and service they need in a coordinated no wrong door approach
4. Promote resilience and safety within the community by ensuring a timely response is provided for adults referred to the team
5. To provide an evidence based, person-centred and holistic initial assessment of barriers
6. Provide a light-touch holistic support for barriers to living in the community and avoiding social isolation
7. Provide a provision of a single joined-up view and pathway into local services to tackle specific needs
8. Ensure multiple referral pathways into and onwards from services across the range of partners to ensure maximum opportunities for people to engage in using local health, social care and voluntary sector service

CANDIDATE REQUIREMENTS

9. To be successful in this role you may have experience working in the following roles: Social Worker / Nurse / Therapist / OT / Occupational Therapy / Physio or similar
10. Educated to DIPSW, BASW / CQSW Or professional qualification (, nurse, OT, Physio), with a degree or equivalent qualification
11. Relevant experience of working within a health, social care or voluntary sector setting
12. Able to build relationships with participants and partners
13. Good report writing skills
14. Experience of supporting people with health conditions and other barriers
15. Good supervisory and communication skills

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us
