We are looking for a reliable part time Care Navigator to join our friendly practice team. You need to flexible and be able to work as part of the team. The hours are 20 hours per week Monday - Friday 2:30pm-6:30pm
As a Care Navigator, you'll be the first point of contact for patients, carers, and healthcare professionals-providing compassionate, efficient, and professional support.
Main duties of the job
* Handle incoming triage, ensuring patients are directed to the most appropriate clinician or service.

We are a friendly GP training practice with over 11500 patients. The team is made up from 6 GP partners, 1 Practice Nurses, 2 HCAs, 1 Practice Manager, 1 Assistant Practice Manager, 4 Administrative assistants, 2 care navigators, 9 Receptionists
Job responsibilities
Making appointments
Giving out results
Ambulance transport bookings
Home visit requests
Contacting other providers
Respond to needs of doctors and nurses during surgeries
Input and extract information from Practice computer system
Observe health and safety guidelines at all times
Any other reasonable duties as necessary
Person Specification
Qualifications
* Good standard of general education GCSE Mathematics C or above GCSE English C or above
* Experience of working within General Practice
