What you will do:
* Coordinate care for complex cases under the Care Programme Approach (CPA), ensuring tailored support for each service user.
* Develop collaborative, person-centred care plans aligned with individual needs and goals.
* Conduct comprehensive risk assessments and implement effective risk management strategies.
* Safeguard vulnerable adults using legislation including the Mental Health Act, Care Act, and Mental Capacity Act.
* Carry out holistic mental health and social care assessments, including Care Act assessments.
* Work closely with carers, families, and wider support networks to deliver comprehensive care.
* Build strong partnerships with statutory and voluntary agencies to enhance service delivery.
* Liaise with inpatient units to ensure continuity of care across services.
* Maintain accurate, up-to-date records and produce high-quality reports.
What we’re looking for:
* Strong experience supporting adults with psychosis and complex mental health needs.
* Excellent organisational skills with the ability to manage and prioritise a varied caseload.
* Sound knowledge of relevant legislation and safeguarding practices.
