Purpose of the role To encourage and enable clients with ARBI to achieve independence and reach their maximum functional potential To provide high quality specialist support to clients with ARBI who are transitioning from Thresholds residential rehabilitation service to the community Assisting clients with ARBI in maintaining their tenancies to prevent homelessness. Work in collaboration with community services, housing teams, Northern Ireland Housing Executive (NIHE), health professionals and other charitable partners. Delivering ARBI and addiction training to clients, carers and stakeholders Accompany clients to events and appointments if required To enable clients to remain long term abstinence. Main Responsibilities of Outreach Project Worker Core Responsibilities To work in a person-centred approach to help clients achieve self-directed goals. To support and facilitate communication between clients and their families, friends, professionals and stakeholders in relation to maintaining tenancies in the community. To assist with identification and attendance at local activities, programmes and voluntary placements regionally. To participate in relevant risk assessments of the home and other environments where community integration may take place. Users of service To provide one-to-one, specialist support to facilitate person-centred goal attainment. To liaise with all relevant stakeholders to ensure a smooth transition from residential discharge to community living, to maximise opportunity for successful tenancies. To deliver education and training for other agencies, staff, health or housing professionals involved in clients community living plans. To liaise with families pre and post residential discharge, offering guidance where necessary. To liaise directly with external statutory agencies for onward referrals and engage with family members. To support & facilitate user communication with their relatives, friends, professionals in order to ensure the continuity of quality and safety in the provision of support. To be aware of the health and well-being of each user and to make appropriate referrals to other professionals/agencies as required to reduce risk of relapse. Model Development/Quality Improvement Ensure that records and reports are completed in an accurate, timely and confidential manner Preparing monitoring information Additional administration tasks which may be required by the Outreach Lead To support in the ongoing development of new models of best practice for ARBI To maintain an up-to-date knowledge of the evidence base and developments within this specialist field Keep up to date with research and models of practice for ARBI To contribute to in service training, Participation in monthly supervision sessions with the Outreach Lead Ensure participation in mandatory and refresher training, as required by Threshold. To keep a continuous personal development portfolio Other/General Regular attendance and participation in staff meetings as scheduled. Liaise with ARBI staff in the residential unit To work in partnership with others including colleagues in other teams at the service and wider organisation. To ensure safety and relevant legislation, policies, standards and guidance are followed and met. To comply with safeguarding & mental capacity act (MCA) policies and procedures. To undertake any other reasonable duties as requested. Working collaboratively and jointly with other internal departments, to include, Finance, People, Digital and Learning and Development colleagues. Health & Safety Adhere and comply with all Thresholds Health and Safety Policies and Procedures. Report and record any matters, for example defects and / or accidents in relation to any issues in respect of Health & Safety immediately, for the attention of the designated person. Contributing to the overall physical cleanliness and general condition of premises. Adhere to infection control policies Act as a Deputy Fire Warden and / or First Aid Person if designated.
