Livewell Community substance use services are looking to recruit a Band 6 Mental Health Practitioner to join the complex needs team on a 12 month fixed term contract covering maternity leave. This is an exciting opportunity to build your skills with working with clients with co-occurring mental health difficulties and substance use.
The role will involve working with a caseload of clients with co-occurring mental health and substance use difficulties in collaboration with their allocated Harbour worker. In addition to this the team support the Plymouth Alliance working across a variety of settings such as Shekinah, the homeless hostels and Plymouth soup run. The team also support the work with the community detox team.
Substance use services has seen recent investments enabling the service to expand and develop our offer, including the introduction of a psychology team. The team is made up of nurses, social workers, support workers, Non-medical prescribers, Doctors, assistant practitioners and psychology staff.
There is the opportunity for monthly CPD sessions, 6 weekly wellbeing sessions as a team and ongoing clinical supervision and reflective practice in addition to shadowing other services such as Harbour, and precribing services.
Main duties of the job
You will be working with a caseload of clients with co-occurring mental health and substance use difficulties using an outreach model. This role will include working across the Plymouth alliance offering specialist mental health advice and support.
You will be working as part of an MDT to provide evidenced based interventions.
You will be required to undertake CPD, a period of induction and training, including BBV testing, Naloxone training and harm reduction.

Job responsibilities
To work as part of the Complex Needs Team within the HarbourPartnership, as part of an integrated, multi-disciplinary/multi agency team,co-ordinating the care for individuals who have a complex range of mentalhealth and substance misuse needs and other complex comorbidities.
To develop the interface between the Harbour Partnership andother services and agencies within the Plymouth Alliance so that people who useservices and their carers are offered focused and co-ordinated packages of carewhich are appropriate to their needs.
To participate in line management, appraisal, supervision,teaching, recruitment and day-to-day management tasks in relation to your rolewithin team.
To provide and coordinate complex packages of treatmentwhich may include the administration of medicines.
This post is a key part of the mental health offer withinthe system and although, based within Harbour the post holder will beresponsible for managing clinics across the city in a range of settings.
To provide a comprehensive healthcare assessment includingrisk assessment and be responsible for providing and coordinating a recoveryorientated package of care alongside other key services and professionalstakeholders.
To use both Halo and System one electronic case managementsystems and to fulfill obligations in relation to NDTMS and Treatment OutcomeProfile. (TOP).
To provide medicines management and psycho education to agroup of people receiving Opiate substitute therapy alongside other medicinesfor a range of mental health and physical health conditions.
Person Specification
Experience
* 3 years post registration. Experience of working in a substance misuse service including the supervision of others.
* Previous experience of a variety of community and inpatient mental health services. Team management experience. Experience of engaging people who are reluctant to use services and are ambivalent to change. Working with a range of prescribers, providers and organizations to promote recovery.
Knowledge
* Mental Health Act and Mental Capacity Act knowledge. Advanced clinical skills with experience and confidence in working autonomously. Knowledge of clinical governance Knowledge of best practice models and an understanding of complex co morbidities, in the field of substance misuse and mental health. Knowledge of both adult and childrens safeguarding.
* Knowledge of recent government developments in mental health and substance misuse. Knowledge of current issues affecting young adults. Knowledge of the breadth and scope of community and inpatient resources for this client group. Knowledge of a range of NICE compliant psychosocial interventions including: solution focused techniques, motivational and engagement enhancement, CBT and goal setting towards the reduction of substance misuse and recover orientated outcomes.
Qualifications
* RMN/Dip HE/BSc Mental Health Nursing- NMC Registration.
* Registered Occupational Therapist Social work degree
* Occupational therapist qualification
* Specialist post-qualification course in substance misuse. Mentorship PGCE or equivalent teaching and assessing in clinical practice
