Rehabilitation Coordinator – Gloucestershire County Council
Location: Great Western Court, Gloucester
Contract: 6-Month Temporary (2 positions available)
Pay Rate: £20.05 per hour
Hours: 37.5 per week (variable shift pattern across 7 days)
Shifts: Early (07:30–15:30) or Late (15:30–22:00)
About the Role We’re recruiting on behalf of Gloucestershire County Council for two Rehabilitation Coordinators to join the team at Great Western Court — a service supporting individuals through their rehabilitation and care programmes following hospital discharge or community admission.
You’ll lead shifts within a multi-disciplinary team, ensuring residents receive high-quality care and rehabilitation support. This role involves administering medication, implementing therapy-led goals, and promoting independence in a positive and caring environment.
Key Responsibilities
1. Lead shifts and coordinate rehabilitation programmes for individuals
2. Administer, prompt, or assist with medication safely and accurately
3. Support residents with personal care and mobility in line with care plans and risk assessments
4. Work closely with the therapy and care teams to review and update plans
5. Maintain detailed written and electronic care records
6. Manage admissions and discharges smoothly, encouraging independence and dignity
7. Supervise Rehabilitation Assistants and act as Officer in Charge when required
Requirements
Enhanced DBS (Adults Barred List)
Level 3 qualification in Health & Social Care (or equivalent experience)
Experience administering medication and supporting therapy-led rehabilitation
A positive, practical, and team-focused approach
Excellent communication and organisational skills
