The Newark and Sherwood Social Work Community Team is seeking a qualified social worker to support people in the community in line with the Nottinghamshire Plan. We are building on our Place‑Based Model and developing key links with local partners and voluntary agencies, and we need someone eager to help us build on our progress.
This role involves working with people from 18 onwards who have needs as per the Care Act 2014, as well as carers and informal carers. The role may also include working with children such as Young Carers, so an enhanced DBS is required. The team is split into Newark North, Newark South and Sherwood; the role will be line‑managed by the Sherwood sub‑team, but you may also undertake work in other areas where demand arises.
Key Responsibilities
* Conduct Care Act assessments, support planning and risk assessments
* Work creatively with adults to promote independence, wellbeing and strengths‑based practice
* Collaborate with external agencies in a hybrid working environment (part office, part home‑based)
* Undertake Mental Capacity Assessments, best‑interest decisions and work set by the Court of Protection
* Complete Risk Enablement Plans and assessments
* Participate in the duty rota system
* Complete enquiries under Section 42 of the Care Act
* Attend MDTs with organisations and partner agencies
Essential Requirements
* Registered with Social Work England
* In‑depth knowledge of the Care Act 2014 and the Mental Capacity Act 2005
* Effective communication, negotiation, organisational and IT skills
* Ability to work autonomously and manage a varied workload
* Commitment to inter‑professional collaboration
* UK driving licence and access to a vehicle (taxi service available if required due to disability)
