Job overview
The Mental Health Service for Older People in Rotherham offers inpatient and community services to people over the age of 65 who present with a range of mental health difficulties and to include those who have a diagnosis of dementia. We also offer specialist services such as a young onset dementia service.
An opportunity has arisen within the multi- disciplinary Community Mental Health Team for an experienced Mental Health Practitioner to join our forward thinking and friendly team.
The role includes specialist nursing and occuaptional assessment, planning and intervention to promote well-being and recovery. The successful applicant will be required to hold and integrate both occupational therapy/nursing specific and generic elements of the job, including carrying out initial full needs assessments and care coordination.
The post holder should be a confident and motivated practitioner, with excellent communication skills, and committed to improving the quality of services provided. A relevant range of experience of working with older people with mental health problems, particularly dementia is required.
You should also have experience of supervising support staff, junior staff and students and be able to demonstrate evidence of further training relevant to this clinical area. You will be a good team worker and an interest in education and research will be supported and encouraged. Other requirements include State Registration (NMC,HCPC) and ability to travel.
Main duties of the job
· To be responsible for the assessment, provision and evaluation of planned nursing intervention and care of older people with mental health problems in the community/in-reach General Hospital Wards, encouraging and developing family and community support whenever possible and appropriate.
· To provide a professional, competent mental health service to older people in Rotherham.
· To participate in the provision for an accessible specialist clinical resource to GPs, members of the Primary Health Care Team, other agencies and giving professional advice on all matters pertaining to the nursing management of the older person with mental health problems.
· To assess the mental health needs of the older person. The holistic assessment will include psychological, emotional, social and physical needs.
· To implement and evaluate planned programmes of care, along with the service user and their carer, as appropriate, taking into account risk management.
· To provide support for carers, signposting where appropriate.

Qualifications
* Registered OT or RMN
Experience
* Working with older people with mental health problems and / or adults with complex emotional needs / personality disorder.
* Knowledge of appropriate legislation
* Ability to work as part of a team and on own initiative.
* Evidence of substantial experience as a qualified clinician
