ASC Social Worker

Responsibilities
* Conduct comprehensive assessments of clients in accordance with agency standards.
* Develop and implement individualized service plans to meet clients\' needs.
* Provide counseling and support to individuals and families dealing with social and emotional challenges.
* Collaborate with healthcare professionals, community organizations, and other stakeholders to facilitate service delivery.
* Advocate for clients\' rights and access to necessary resources and services.
* Monitor and evaluate clients\' progress and adjust service plans as needed.
* Document all client interactions and maintain accurate case records in compliance with regulatory requirements.
Qualifications
* Master\'s degree in Social Work (MSW) from an accredited program.
* Current Licensed Clinical Social Worker (LCSW) or equivalent licensure in the state.
* Experience working with diverse populations in a social work or healthcare setting.
* Strong knowledge of social work principles, practices, and applicable laws.
* Excellent interpersonal and communication skills, both written and verbal.
* Ability to work collaboratively in a multidisciplinary team environment.
* Commitment to ongoing professional development and staying updated on best practices.
